This page will detail the steps to follow should you have an injury at a game or practice that is serious or appears to be serious.
Injuries at a game
It is each coaches' responsibility to ensure that all potentially serious injuries that occur at a game are noted in as much detail as possible on the game card. We realize that you may not be able to identify the severity of the injury at the field. However, a note on the game card like "Allison Manns of Berkeley Heights sustained a blow to the head at the beginning of the second half. She was helped off the field and did not return to play." is very helpful.
The ICYSL does not use game cards. All injuries must be reported via e-mail. Coaches should send the details of the injury (game number, teams and age of teams, location, date and description of injury to [email protected] and CC David Tarendash ([email protected]) and Sean Manns ([email protected]).
Injuries at a coach or trainer practice
As soon as you are aware of a practice injury you should send an e-mail to Sean Manns ([email protected]) to report what occurred, who witnessed the injury and the date and time. This description should be written by a carded volunteer or non-volunteer coach present at the practice. If your trainer was the only carded soccer coach there, please encourage them to write to Sean directly. He will retain the information in the event that a claim must be filed.
The Club believes that it is much more likely that claims will be honored if these reports are generated by trainers listed on the team's official roster. As a reminder, if you want to add your trainer to your roster, you need to provide the following:
* Copy of existing stamped roster
* Copy front and back of trainers card
* Copy of New SAGE form, coaches version, signed by trainer
* At least two copies of the new, unstamped roster, including the trainer
